Life History & Review

The Life History & Review is a very detailed review about your loved one’s past, their interests, preferences, motivations and dislikes. Every individual is different and in turn the relevant parts of the Life History Review will vary from person to person, based on what type of person they are and the way they have lived their lives.

Our team will use the Life History & Review questionnaire to complete the Life Services Plan that our team will follow when caring for your loved one. With time and once we have begun to build a personal relationship with your family member, our plan of care may change.  However, initially this information is key to understanding and knowing how to begin caring for your loved one.

Tips for Completing the Life History Review:

  • If possible, put aside some time where you can quietly reflect on the life of your family member when completing the Life History Review.
  • Put a lot of detail into the areas of the questionnaire which are relevant to your loved one. For example, listing favorite smells will not be at all important to some but for others there won’t be enough room in this section to list them all. We are interested in the most timely and factual information about your family member.
  • Have different family members contribute to completing the Life History Review. Different family members have different relationships with your loved one and their own unique perspective that can help to paint a full picture. So take advantage of it! Pass the Life History & Review around between family members or ask us for additional copies.
  • If it is important to you, write it down. If there is something that stands out to you as being important to your loved one, write it down! If your loved one has a certain routine for brushing their teeth, a morning or late night ritual, favorite TV shows, or favorite songs, include that too!

We ask so many details about the past because understanding the past of your loved one may help us to understand what is going on with them now, particularly if they are having difficulty expressing themselves verbally or are initially displaying behaviors that don’t make a lot of sense. For others, this isn’t at all true and the person they are projecting themselves as has no similarity to who they were earlier in life. These are important things to establish and understand.
