Building Collaborative Partnerships

Our ultimate goal is to create a collaborative partnership among people with dementia, their families, medical professionals, and our care staff that will enhance each resident’s daily life experiences and the quality of support and care they receive.

As part of the process to achieving this goal, we use our extensive Life History Interview and pre-move-in assessment process to collect information from family, friends, and health professionals.  This information helps us understand each resident’s physical, emotional, spiritual, and social needs along with the distinctive nuances of their personality and the meaning behind their behaviors.

Exploring each aspect of our resident’s lives and the personal experiences they have shared with their families over the years is where we begin to put together the ideas that allow us to create a personalized memory care program  that will meet our resident’s changing needs. It is also from this information that we will develop the Life Services Plan  that guide every care team member when caring for your loved one.

Interactive Relationships with Families

Another very important goal for us is to have an open, ongoing, interactive relationship our resident’s key family members.  They offer us personal insight into the things that bring joy and happiness to their loved one along with the things that can make them uncomfortable or upset.

At The Heritage, we offer regular opportunities for communication between families and our care team.  Based on the expectations of the family, we will provide regular updates through team meetings, phone calls, emails, and face to face discussions.  Through these interactions, we strive to not only give information to the family member, but also to solicit feedback from their perspective.

This communication is critical to our success in continuing to better understand our residents and how to create the most supportive environment for each person.  We know that the love and support of a family member or friend can be very strong, and we want to add our care and support to yours!
