Life Services Plan
Each resident at The Heritage is unique and has a personalized plan of care. This plan of care is called the Life Services Plan. This care plan identifies each area of support that a person may need, how those needs will be met, and what the plan is to meet those needs. It is tailored to each person based on their individual needs and desires.
The basic elements of the Life Services Plan identify the level of support or assistance needed in the following areas as identified through our functional capacity assessment:
